Provide enough information to locate the policy

Include the named insured, policy or account information when available, vehicle, driver, preferred delivery method, and the reason a replacement is needed.

Do not use the form for an unconfirmed coverage change

Adding a vehicle or driver, reinstating a policy, or changing coverage requires agency or carrier confirmation. A replacement-card request cannot make an unapproved change effective.
